When You Need a Private GP for Death Certification
When a loved one passes away at home, particularly outside of normal NHS working hours, the process of obtaining a death certificate can feel overwhelming. A private GP can attend the home to verify the death, issue the Medical Certificate of Cause of Death (MCCD), and complete the cremation paperwork, often on the same day. This is especially important when funeral arrangements are time sensitive, as is the case for families observing religious customs that require burial or cremation within 24 hours.
What Is a Medical Certificate of Cause of Death?
The MCCD is the official document that records the cause of death. It must be completed by a registered medical practitioner who either attended the deceased during their final illness or who has sufficient knowledge of the medical history to determine the cause of death. The certificate is then submitted to the Registrar of Births, Deaths and Marriages, who issues the formal death certificate that the family needs to proceed with funeral arrangements.
The Verification and Certification Process
When we attend a home following a death, the process involves verification of death, which is a clinical confirmation that the person has died; documentation of the circumstances, including a review of the medical history and recent treatment; completion of the MCCD with the cause of death listed in the required format; and completion of the medical examiner’s notification form, which is now a statutory requirement for all deaths in England.
If cremation is planned rather than burial, an additional form is required. This form asks the certifying doctor a series of questions about the circumstances of the death and requires confirmation that there is no reason to refer the death to the coroner.
What Information the Family Needs to Provide
To complete the paperwork efficiently, we need the full name, date of birth, and home address of the deceased; their NHS number (if known); details of their medical history, recent hospital admissions, and current medications; the name and contact details of their NHS GP and any specialists involved in their care; and, if available, copies of recent hospital discharge letters, scan results, or outpatient correspondence.
If the deceased was under the care of a palliative team, hospice, or oncologist, any written communication confirming a do not resuscitate (DNR) order or palliative care plan is extremely helpful.
The Medical Examiner Process
Since the introduction of the medical examiner system in England, all deaths require scrutiny by a medical examiner before the MCCD can be registered. The medical examiner reviews the proposed cause of death, checks it is consistent with the medical records, and contacts the next of kin to discuss the circumstances. This process usually takes a few hours but can be expedited in urgent cases, particularly when there are religious requirements for prompt burial or cremation.

When a Death Must Be Referred to the Coroner
Not all deaths can be certified by a GP. A death must be referred to the coroner if the cause of death is unknown or uncertain, the death was violent or unnatural, the death occurred during or shortly after surgery, or the deceased was not seen by a doctor within a specified period before death. In these circumstances, the coroner will decide whether a post mortem examination or inquest is required before a death certificate can be issued.
