There was an internal urethral itch which was particularly distressing. They had tried an antifungal cream without improvement. There was no visible rash on examination, but the constant itching was significantly affecting their mental state.
Rather than simply prescribing another cream, Dr Parmar took a thorough history, performed a careful examination, arranged a urine test to exclude a urinary tract infection, prescribed appropriate topical treatment and a barrier lotion, advised avoiding soap in the area (which can worsen irritation), and provided information about a specialist sexual health clinic should the patient wish to pursue further STI testing.
