What Does Gallbladder Pain Feel Like?

A patient attended Clinique Alpa with a 2 year history of epigastric pain and bloating that occurred approximately 30 minutes after eating any food. The bloating was significant. The pain could wake them from sleep. Symptoms improved completely when not eating, and the patient had noticed marked improvement during fasting periods.
There was no weight loss, no difficulty swallowing, no reflux, no vomiting, and no blood in the stools. Previous treatment with a proton pump inhibitor had helped temporarily, but symptoms always returned. Importantly, tests for Helicobacter pylori, faecal immunochemical testing, and calprotectin had all come back negative, which ruled out several common causes.
We arranged an ultrasound which revealed a contracted gallbladder containing several stones, the largest measuring 2.4 centimetres. The liver showed mild fatty changes but was otherwise normal. The kidneys, spleen, and aorta were unremarkable. On examination, there was right upper quadrant tenderness consistent with the imaging findings.
The diagnosis was cholelithiasis, gallstones. We arranged an urgent referral to an upper gastrointestinal surgeon for cholecystectomy, repeated the baseline blood work, and prescribed interim medication for symptom relief. Critically, we also provided safety netting about cholecystitis: if the pain became severe, constant, and accompanied by fever, the patient should attend hospital immediately as this would suggest the gallbladder had become inflamed or infected.
