We recently assessed a patient with severe lumbar spine disc prolapse causing nerve impingement. The nerve compression had led to bladder and bowel incontinence. On assessment, this patient was unable to get on or off the toilet without physical assistance. His back would seize during transfers, leaving him stranded. There were occasions where he soiled himself because help was not available quickly enough.
Our care plan included timed visits coordinated with his medication schedule, continence management support, assistance with washing, dressing, and toileting, and direct clinical oversight from Dr Parmar who prescribed pregabalin for the neuropathic pain and arranged an updated MRI. The carer and the doctor worked as a team — not in separate silos.
