From Our Practice: Severe Back Pain with Leg Symptoms
A patient attended with severe back pain that had been present at a low level for 2 to 3 months but had worsened dramatically over the previous 4 to 6 days. The pain was radiating down the right leg, and she was unable to lie flat, sleep properly, or move without significant discomfort. Previous treatments including exercises, topical creams, and over the counter painkillers had provided no relief. An MRI scan performed 2 years earlier had identified some changes, but the current symptoms represented a significant deterioration.
On examination, there was localised tenderness in the lower back with marked muscle tightness. Importantly, bowel and bladder function were normal, and sensation was preserved. The doctor prescribed a nerve pain medication to target the sciatic component and a muscle relaxant to ease the spasm. A new MRI scan was arranged to assess for any progression of the spinal changes. Clear safety netting advice was provided: if the patient lost sensation when going to the toilet or when passing urine, she was to call emergency services immediately, as this could indicate spinal cord compression requiring urgent surgical intervention.
