From Our Practice: When Fasting Meets Medication Crisis
We assessed a patient during Ramadan who presented with severe lumbar spine pain radiating down the right leg. His MRI had previously confirmed a disc prolapse with nerve impingement, and his mobility had deteriorated to a maximum of twenty metres. He was experiencing bladder and bowel incontinence secondary to the nerve compression.
Critically, the patient was fasting and had not taken any medications — including his regular painkillers — for the entire day. By evening, the pain was excruciating. He could not drive, could not sit on public transport, and was alone in his flat with no one to help him.
An out-of-hours home visit allowed Dr Parmar to assess the severity of the situation in the patient’s own home, observe his functional limitations directly, prescribe pregabalin for the neuropathic pain component, arrange an urgent MRI, and provide clear guidance about which medications could and could not be taken during fasting hours — a conversation that required cultural sensitivity and clinical pragmatism in equal measure.
