From Our Clinic: A Presentation That Could Have Been Missed
A patient attended recently with a sore throat and both tonsils visibly enlarged. They reported difficulty breathing when lying down at night — not from a respiratory problem, but because the tonsils were so swollen they were physically reducing the airway space. The patient also had a persistent dry cough attributed to vaping, which complicated the clinical picture.
On examination, the tonsils were very inflamed and enlarged bilaterally, though no exudate had formed yet — suggesting we had caught the infection early. The chest was clear. We prescribed antibiotics, appropriate pain relief, and advised on hydration. Critically, we also discussed the vaping — a factor many patients do not associate with throat problems, but which causes chronic irritation and can slow recovery from infections.
A telephone triage appointment would not have captured the airway concern. This patient needed to be seen, examined, and treated on the same day.
