Fit notes and health at work

Overview

Health and work conversations are about function as well as symptoms. The relevant questions include what your work involves, which tasks are difficult and whether temporary changes could help. A fit note records a healthcare professional’s assessment; it is not simply a document issued on request.

For shorter sickness absences, self-certification is usually the first step. NHS guidance explains when a fit note is needed. Before paying for an appointment, check whether the clinician already treating you can provide the appropriate evidence and what your employer is actually asking for.

By telephone

A telephone assessment can be suitable for discussing the health problem, available records and its effect on work. The clinician may consider whether you are not fit for work or whether support could make work possible. The information needed depends on the circumstances.

Explain what is difficult in concrete terms: concentrating, travelling, physical tasks, attendance or interaction with others. You can also discuss recovery and the support already offered. Ask how any advice should be taken forward with your employer.

What cannot be done remotely

The doctor may need further records or examination before reaching an opinion. Neither a requested wording nor a requested period of absence can be assumed in advance. This page does not establish that every form of employment certification is available through the service.

An ordinary fit note is different from a specialist occupational assessment, driving medical or formal declaration for a safety-critical role. Where specific standards or forms apply, the appropriate assessing service may be needed. Employment disputes and benefits entitlement need their own advice.

What to prepare

  • Clarify whether the request is for a fit note, report or occupational assessment.
  • Describe the main demands of your work and current functional difficulties.
  • Have relevant treatment information and existing certification available.
  • Explain changes or support already discussed with the employer.
