Live in Care Cost Breakdown, No Hidden Extras
Written by Dr Mitesh Parmar, MBBS MRCGP. Published 29 August 2026.
The first call should not invent a number to win the enquiry. A son asks what it costs. A sister has already written a figure on a pad, copied from a national site. Live in care with Clinique Alpa starts at £1,650 a week for one person, subject to assessment. That is the starting point, not the quote for this house. A live in care cost breakdown that is honest comes after someone has actually looked.
The question they ask is how much. The question they mean is whether they can do this without emptying the savings, and without a second bill arriving in week three.
Nights, couples, and heavier support change the fee. So does how quickly care has to start. VAT is a fair question. So is what happens if needs go up.
What a live in care cost breakdown has to show
Ask what is included: the carer, cover, the written plan, reviews, management. Ask what is not: waking night cover, a second carer, the carer's travel at the start of a placement if that applies. The household provides the carer's meals and a private bedroom. There should be no extras that were obvious at assessment and absent from the quote.
Ask any provider whether tax is added on top, because a number that looks lower on a first call can be a different number once tax is sitting on top of it.
Families fund this privately. We do not hold local authority or NHS contracts, so we cannot invoice a council or an integrated care board on your behalf. Attendance Allowance can be part of how a family pays. It will not cover a live in fee. NHS Continuing Healthcare is decided by the NHS, not by us. If it is awarded, it can fund care at home. If it is not, that is not a judgement on whether the person needs help. Direct payments from a council are a question for the council, because we work privately.
Compare a real quote with a real care home's fees rather than two headlines. For one person with significant needs the two are often in a similar range. For a couple who would otherwise pay two care home fees, live in care is usually the less expensive option and the one that keeps them together. A care home is still sometimes the better choice, particularly where nursing is needed or the house cannot be made workable. We will tell you if we think so.
What we actually look at when a family calls
What families tell us is that they wanted the number before they wanted the visit, because the number is what the sibling conversation is waiting on. We can give the starting figure on that first call. We cannot give the quote, because the quote is the house, the nights, and whether this is one person or two.
We walk the home. Stairs, bathroom, the chair, whether a carer has a private bedroom. Nights, because broken nights are a different product from a carer who sleeps. Medicines, mobility, memory, mood, appetite, continence. What a good afternoon still looks like. The family rota, and how close it is to breaking. Whether waking night cover or a second carer is in the picture, because that belongs on the page, not as a surprise.
A doctor from our team carries out that assessment. Most domiciliary providers have no clinician anywhere near the care plan. That is the functional difference. The carer who then moves in is not a doctor and not a nurse. The assessment is charged separately. If you then start a long term live in care package, the fee is credited against the first month. Clinical work stays with the NHS. We work alongside it. We do not replace it.
You get the breakdown in writing. Read the published figures on the costs page before the call if you want the starting point in front of you.
When the starting figure will not hold
A starting fee is for needs at the more straightforward end of personal care. It is not a promise that this house sits there. The signals that the quote will sit higher, or that live in care is the wrong product, are practical.
- Nights are already broken, or likely to be, after a discharge or a fall.
- Two people in the household both need support.
- Needs are complex: advanced memory problems, Parkinson's with timed medicines, care after a stroke.
- The house cannot take a carer with a private bedroom, which is not a fee problem, it is a no.
- The work being asked for is nursing, not personal care.
- The family is hoping the starting figure will stretch, without anyone walking the rooms.
When to call us sooner is when you are already comparing screenshots of weekly fees and have not yet said what 2 in the morning is like. The number that looks lowest is often the one that assumed a night that does not exist in this house.
How care is arranged here
Clinique Alpa is a CQC regulated domiciliary care provider. We work across Enfield, Edmonton and the surrounding boroughs. The first conversation is with a manager, not a call centre, and there is no obligation. If live in care is unlikely to suit, we say so, including when the honest cost will not be the starting figure, or when a care home is the better product.
If we go ahead, the person is assessed and we write the care plan with you. You meet the carer before care begins. We match on need and experience first, then on how the household runs. We do not promise one named carer for ever. Cover for days off is arranged by the office, and that cover is part of what the weekly fee is for. You should not be left to find a replacement yourself.
If needs go up, the plan is reviewed and the fee is discussed in writing before it changes. It should not drift. The first fortnight is where the house confirms or contradicts the assessment. That is also when to say if the match is wrong.
What the carer actually does sits on the live in care page. The sequence sits on how it works.
You are allowed to ask what is not included. That question is not awkward. It is the job.
