From Our Practice: A Chest Infection Complicated by Cardiac Anxiety
A patient presented with chest symptoms including a heavy chest feeling, sore throat, congestion, and chest pain over 4 to 5 days. The pain was in the middle and left side of the chest, with wheezing. There was no coughing up blood. The patient also had a long history of acid reflux, had eaten spicy food recently, and was experiencing diarrhoea.
What made this consultation particularly important was the patient’s anxiety about their heart. They were worried that the chest pain was cardiac rather than respiratory. They had an antibiotic allergy, which limited the options. Recent blood tests showed a normal white cell count (arguing against severe bacterial infection), very low vitamin D, normal cholesterol, and normal glucose.
We diagnosed a chest infection with possible concurrent gastroenteritis, started an appropriate antibiotic that was safe given the allergy, arranged an echocardiogram and 24 hour ECG to provide definitive reassurance about the heart, organised a stool antigen test for Helicobacter pylori given the reflux history, and started vitamin D supplementation for the deficiency. The patient left with treatment for the infection, investigation of the cardiac concern, and a plan for the reflux, all addressed in a single 30 minute consultation.

When Do You Need Antibiotics for a Chest Infection?
Not every chest infection needs antibiotics. Many are viral, and antibiotics will not help. You are more likely to need antibiotics if you have a productive cough with green or yellow sputum, a fever that has lasted more than 3 days, your breathing has become noticeably worse, you have an underlying condition such as asthma, COPD, or diabetes, or you are over 65.
